In the event of an emergency, every attempt will be made to contact the parents if a student requires immediate medical attention. The emergency card on file in the school office lists whom to contact in case of any emergency. School staff will follow the parents' direction insofar as it is possible to do so. You are urged to check your own insurance or consider the student insurance available.
PLEASE FILL OUT THE EMERGENCY CARD COMPLETELY. IF WE HAVE AN EMERGENCY IT IS IMPORTANT WE HAVE A NUMBER TO REACH YOU. NOTIFY THE SCHOOL IN WRITING IMMEDIATELY IF ANY CHANGES ARE TO BE MADE ON THE EMERGENCY CARD.
ONLY THOSE PERSONS LISTED ON THE EMERGENCY CARD WILL BE PERMITTED TO TAKE STUDENTS FROM SCHOOL.
Health screenings are state mandated for 7th and 8th grade students. Hearing and vision screening for 7th grade girls and 8th grade boys.
It is California State Law that students may NOT bring medication of any kind (including over-the counter drugs) to school without both doctor and parental written permission.

Doctor's note for readmission- Pupils returning to school with casts, crutches, or braces must have a doctor's note for readmission to school. It should indicate the length of time the pupil will be restricted from regular activities, such as P.E. A doctor's note is also requested for students returning after a serious illness, injury, or hospitalization.
