As a leadership employee, you quality for benefits. You MUST sign-up for coverage within 30 days of your hire/qualifying date. You do not have the option to opt-out of coverage.


    In the below package you will find forms that need to be completed in order for you to receive benefits. Turn in the necessary forms to the Benefits desk at the District Office. If you are adding dependents to your plan please include the following:

    • Spouse/Domestic Partner: first page of your taxes with "married filing jointly, or married filing separate", and a copy of your marriage certificate or SISC notarized domestic partnership certificate.
    • Child: a copy of every child's birth certificate.

    YOU MUST FILL OUT THE ENTIRE PACKET.  (If you are not electing Kaiser, you do not need to fill out the Kaiser form).

    Once you have completed the packet you may drop off the documents Monday - Friday, between 8:00 a.m. and 4:30 p.m.
